# US Orphan Drug Market Reaches USD 224.9 Billion by 2033 as Half of 2025's Novel Approvals Carry Orphan Designation

**25 October 2026 — The US orphan drug market reaches USD 224.9 billion by 2033, growing at 4.54 per cent from USD 157.7 billion in 2025, according to a new report from Pheonix Research. The finding that frames the forecast is not the revenue line. It is the evidence underneath it.**

The FDA's Center for Drug Evaluation and Research approved 46 novel drugs in 2025, of which 23 — exactly 50.0 per cent — carried Orphan Drug Designation. Between them, those 23 programmes were supported by **5,001 patients** in the pivotal trials that took them to approval: 19.2 per cent of the roughly 26,000 pivotal participants behind all 46 approvals, at a mean of 217 and a **median of 114 patients per programme**.

Nine of the twenty-three were approved on a data set of a hundred patients or fewer. Forzinity (elamipretide), approved in September for Barth syndrome, rested on twelve. Three *non-orphan* programmes in the same year — Enflonsia, Journavx and Lerochol — enrolled 9,279 patients between them, which is **1.86 times the entire orphan cohort**.

"So the premise is right on both halves. But a count is not a market."

"The evidence base underneath those 23 approvals was 5,001 pivotal-trial participants — 19.2 per cent of the roughly 26,000 participants supporting all 46 approvals, at a mean of 217 and a median of 114 patients per orphan programme."

### The forecast: three paths, and a deliberately wide band

 | Path | | 2025 (USD bn) | | 2033 (USD bn) | | CAGR | | Assumptions |

 | Low | | 157.7 | | 170.7 | | 1.00% | | Orphan share of approvals flat at 50%, patients +1.0%, net price +0.0% |

 | **Base** | | **157.7** | | **224.9** | | **4.54%** | | Share 50.0→54.0%, treated patients +1.5%, net price +2.0% |

 | High | | 157.7 | | 264.3 | | 6.67% | | Share +1.1 points a year, patients +2.0%, net price +3.5% |

"The spread between low and high is USD 93.6 billion, which is 59 per cent of the base year: for a market whose unit economics are this levered to two variables, that is the honest width of the answer."

The base year is derived rather than observed, and the report publishes the chain: approximately USD 263 billion of 2024 US specialty drug spending, of which 54 per cent of pharmaceutical spending is now specialty, multiplied by the 55 per cent orphan share of specialty drug-indication pairs, grown 9.0 per cent to 2025. A reader who rejects either input can rebuild the forecast from their own.

### Prices stopped cooperating in 2025, for the first time in the series

ICER and West Health put the 2025 median annual *net* launch price at **USD 257,000** against a median list price of USD 293,000. That is up 35 per cent on USD 190,000 in 2022 — 10.59 per cent a year — but **down 2 per cent on 2024, the first decline in the series**. Roughly a quarter of launches priced above USD 500,000 net, and a further 34 per cent between USD 200,000 and USD 500,000.

"Price is the second multiplier, and it is mechanically linked to the first: the smaller the population, the higher the price a payer will tolerate."

The report offers a second reading and declines to present it as settled: route of administration predicts price better than the science does — oral USD 314,000, subcutaneous USD 449,000, intravenous USD 517,000 — and twelve of the twenty-three 2025 orphan approvals are oral solids. A cohort that oral-weighted should price below the series median, and it did.

### The second rare label became the whole strategy

Section 71203 of the Working Families Tax Cuts Act (Public Law 119-21) widened the Medicare negotiation orphan exclusion: a product is now excluded if *all* its approved indications are rare, however many there are, rather than only if it holds a single indication. CMS addressed the exclusion in final guidance for IPAY 2028, published 30 September 2025.

"For a sponsor choosing between a broad label and a stacked rare-disease label, the 2025 policy change makes the stacked rare label the higher-net-present-value choice."

The report treats the floor as less settled than the statute reads. The IPAY 2028 guidance still describes the narrower single-designation test, and its comment record shows CMS replying that it cannot amend the statutory requirement. The statute is broader than the policy that articulates it, and a second rare indication — now worth both revenue expansion and permanent re-affirmation of the exclusion — is the highest-value position in the market. The base rate for follow-on activity is 19.9 per cent of specialty drug-indication pairs (188 of 947), at a mean of 1.7 per product.

### Approval is not the binding constraint; fill is

About one third of outpatient prescriptions filled within 30 days in the first post-approval year, with commercial coverage at 51 per cent, Medicaid at 42 per cent and **Medicare at 20 per cent**; non-coverage rejection runs at 58 per cent in Medicare against 24 per cent commercial. At the median net price, a filled patient-year is worth USD 257,000 and a written prescription is worth **USD 85,658**.

"At a median net price of USD 257,000, the difference between a 33.3 per cent fill and a 51 per cent commercial-only fill is USD 45,442 per prescription written. No pricing negotiation in the period covered by this report is worth as much as that gap."

The cohort's therapeutic mix maps onto that channel split with unusual precision: acromegaly, generalised myasthenia gravis, idiopathic pulmonary fibrosis, multiple myeloma and obstructive hypertrophic cardiomyopathy are Medicare-weighted populations and carry five of the cohort's larger programmes.

### About the report

**US Orphan Drug Market Report, Size, Share and Forecast 2026–2033: Half of 2025's Novel Approvals Were Orphan Drugs and Carried a Fifth of the Evidence** analyses all 23 orphan-designated novel approvals of 2025 with their pivotal programme sizes, segments the cohort five ways — therapeutic area, route of administration, evidence scale, payer channel and indication breadth — and sets out the value chain, competitive structure, investment case, technology-price relationship, regulatory stack and five risks each with a named falsifier.
