Mexico Apixaban Market Reaches MXN 5,676.0 Million in 2025 as Generic Entry Cuts the Shelf Price 72.9 Per Cent - Pheonix Research

· Pharmaceutical

MEXICO CITY — October 2026 — Generic apixabán reached Mexican pharmacy chains in 2026 at MXN 598 for a 60-tablet box, against MXN 2,205 for the innovator brand Elicuis on the same shelf. That is 72.9 per cent lower, and it is the first price movement the molecule has recorded in Mexico since launch.

Pheonix Research sizes the Mexican apixaban market at MXN 5,676.0 million, or USD 303.5 million, in 2025, falling to MXN 4,359.7 million, or USD 233.1 million, by 2033. Treated patient-years rise from 236,500 to 418,000 over the same period, a 76.7 per cent increase, while revenue falls 23.2 per cent. Volume compounds at 7.38 per cent a year and revenue compounds down at 3.24 per cent, because revenue per patient-year drops from MXN 24,000 to MXN 10,430.

The box is a month, so the shelf price is an annual price

Apixabán is dosed at 5 mg twice daily, which makes a 60-tablet box exactly one month of therapy and twelve boxes exactly one patient-year. That arithmetic turns a retail listing into a market price rather than an anecdote: MXN 7,176 a patient-year for the generic against MXN 26,460 for the innovator product, read off current chain listings and not from a modelled national average.

The same substitution that produced those listings is the mechanism the global generic drug market describes at the level of the whole portfolio: the price step happens at the point of registration, and the volume response follows it with a lag measured in quarters.

The market splits in two, and only one half cleared

Channel Patient-years, 2025 Revenue per patient-year Value, 2025
Retail 141,900 MXN 28,800 MXN 4,086.7m
Institutional 94,600 MXN 16,800 MXN 1,589.3m
Blended 236,500 MXN 24,000 MXN 5,676.0m

Generic manufacturers hold COFEPRIS registrations and have won their court rulings, but a further innovator amparo keeps them out of public procurement — the channel that carries 40.0 per cent of treated patient-years. The counter is open and the tender is not.

The consequence is the largest single number in the report. Institutional purchasing stays on innovator pricing at MXN 16,800 a patient-year while retail already clears at the generic rung, so the institutional channel transacts MXN 1,589.3 million a year — 28.0 per cent of the entire market — for 94,600 patient-years that would cost MXN 678.8 million at the counter price. The MXN 910.5 million difference is what the procurement block is worth each year.

The retail premium is the thing generic entry destroys

Retail carried a 1.71 times price premium over institutional purchasing in 2025. By 2033 it carries 1.10 times. The generic wave does not only cut the retail price; it removes the reason the retail price was higher, because a chain that can source a registered generic no longer has to recover an innovator’s margin at the counter.

The forecast runs on the observed shelf ladder rather than on a single curve. The generic rung is held at the current chain private-label price of MXN 7,176 a patient-year. The branded rung declines from MXN 23,300 to MXN 19,700. The two are blended by a generic share rising from 18 per cent in 2026 to 74 per cent in 2033, which produces MXN 20,400 falling to MXN 10,430. The global statin market shows the same shape from further along the curve: the post-patent decade is a volume story wearing a price story’s clothes.

Volume is the signal, revenue is the accounting

The patient base grows on the diagnosed atrial fibrillation population rising from 1.50 million to 1.65 million, anticoagulation coverage from 46 to 58 per cent, and apixabán’s share of oral anticoagulation from 30 to 38 per cent, with venous thromboembolism treatment and post-orthopaedic prophylaxis adding 54,340 patient-years by 2033.

That is why a market losing 23.2 per cent of its peso value is treating 76.7 per cent more people. For a supplier the relevant question is not whether the molecule grows — it does — but which rung of the ladder it is standing on when the generic share reaches three quarters. For the entrant, roughly half of that margin is set upstream at the input contract, which is the subject the active pharmaceutical ingredient market prices.

How the figures were built

The unit of this market is the patient-year: one person on apixabán for twelve months at the standard 5 mg twice-daily dose, which is twelve 60-tablet boxes. The base is the 1.50 million Mexicans with atrial fibrillation published in the joint Mexican position document on the condition, taken through 46 per cent anticoagulation coverage and a 30 per cent apixabán share of oral anticoagulation to 207,000 patient-years, plus 29,500 patient-years from venous thromboembolism treatment and post-orthopaedic prophylaxis, giving 236,500 patient-years in 2025.

Those patient-years are priced channel by channel rather than at a national average: retail at MXN 28,800 per patient-year, which is twelve boxes of the innovator product at the chain list price of MXN 2,205, and institutional at MXN 16,800, reflecting awarded tender pricing. That gives MXN 4,086.7 million and MXN 1,589.3 million, or MXN 5,676.0 million blended at MXN 24,000. The segmentation ties back to the headline on both dimensions.

To 2033 the patient base grows on the atrial fibrillation population rising to 1.65 million, anticoagulation coverage to 58 per cent and apixabán share of oral anticoagulation to 38 per cent, with non-atrial-fibrillation use adding 54,340 patient-years, for 418,000. Pricing runs on the observed shelf ladder described above. Currency is held at the Banco de Mexico rate of MXN 18.7005 to the dollar published in the Diario Oficial on 23 October 2026.
